U.S. Visa Application Process
The U.S. visa application process in Ukraine includes the following steps:
- payment of the consular fee;
- completion of the visa application form on the website;
- preparation of necessary documents and photographs;
- submission of the application to the U.S. embassy;
- scheduling an interview at the U.S. embassy;
- successful completion of the interview and submission of biometric data.

Documents for Applying for a Medical Visa to the U.S.
A visa for treatment in the United States requires the applicant to provide the following documents:
- receipt of payment for the consular fee in the amount of 160 USD;
- a valid passport with sufficient (1-2) blank pages, expiring no earlier than 6 months after the trip;
- completed online DS-160 visa application form (for the interview, only the printed form with the ten-digit barcode confirming submission for review is required);
- photographs meeting U.S. embassy standards (size 5×5 cm, on a white or light gray background, taken within the last 6 months, etc.);
- a letter from your doctor indicating the diagnosis and justifying the need for treatment in America;
- confirmation from the doctor and hospital in the U.S. of their readiness to provide medical services, including information on timing and cost.
Additionally, prepare supplementary documents that may be requested by the consular officer during the interview:
- internal passport;
- tax identification number certificate;
- employment verification document indicating position and salary;
- proof of personal or sponsor financial guarantees sufficient to cover treatment and stay expenses in the U.S.;
- property ownership data;
- marital status information;
- copy of pension certificate (for retirees);
- certificate from an educational institution (for students);
- copy of birth certificate and travel permission from both guardians (for children).
Documents from the second list do not need to be submitted immediately with the main ones. They will be provided upon request by the consular officer.
